Please note: Manufacturer equipment discount forms are strictly for the use of approved USTA program providers that have registered with the USTA as an organizational member in one of the following categories: School/College, Community Tennis Association, Parks & Rec, National Junior Tennis League, Community Service Organization.
Once your request has been processed, you will receive an email containing the requested equipment order form(s) to complete.
To place an equipment order with a specific manufacturer, please do the following steps:
- Fill out the equipment discount sheet provided ($75 minimum order required)
- Save a copy of the order form and email to the address listed on the discount sheet; or print and fax a copy to the fax number provided.
- Orders will be fulfilled within 2-3 weeks.
