Please complete this document and submit it along with copies of receipts by MAY 1, 2022. Your Club must retain receipts of all expenditures for five (5) years. Please scan the receipts and email to districtgrants5150@gmail.com to complete the Final or Interim Report.

Club President confirms that District Grant funds (DDF) were spent only for eligible items in the approved District Grant. All information contained herein is accurate and true.
