I the registrant am legally agreeing to all terms and conditions set forth by the team in which I am registering. Team rules are located under the forms tab.

I agree that I am at least 18 years of age. I further agree that any information provided above is truthful and accurate to the best of my knowledge.

I also state that any falsification of documents, illicit or immoral activities, fighting, etc. will not be tolerated. Such activity will lead to penalties including: partial and/or permanent expulsion from the team, including no reimbursement of funds.

I hereby give my consent to photograph, film, video and then use, reproduce, and publish said images of me. I agree that photographs/negatives, film, and/or video thereof shall constitute the sole property of Oklahoma Tomahawks Lacrosse, with full right of disposition in any manner whatsoever, including the right to publish on the internet. No compensation will be provided. I will be allowed to download a copy of the said content for my private, personal use. I hereby release Oklahoma Tomahawks Lacrosse and his/her legal representatives and assigns from any and all claims whatsoever in connection with the use, reproduction, publication of the images thereof.

To participate on the Team, all Participants, on behalf of themselves, and their parents, spouses, children/wards, heirs, assigns, representatives, estates, successors, attorneys, insurers, and all other persons, firms, partnerships or corporations connected therewith (collectively referred to hereinafter as the "Releasing Parties"), forever, finally, fully, permanently and unconditionally waive, release, acquit and discharge the team, players, its board members, directors, officers, coaches, coaching staffs, officials, volunteers, agents, licensees, sponsors, etc., including present and former, (collectively referred to hereinafter as the "Released Parties"), to the fullest extent permitted by law, from any and all charges, claims, debts, disputes, demands, suits, causes of action, rights of action, dues, sums of money, accounts, liabilities, losses, expenses and damages, absolute or contingent, known or unknown, whether or not asserted, threatened, alleged or litigated, now existing or arising in the future, at law or equity, whether caused by the negligence of Released Parties or otherwise, that arise out of or relate in any way either directly or indirectly from my participation in activities including, but not necessarily limited to, tournaments, games, pickup games, practice games, practices, transportation to and from games/tournaments or otherwise, the use of practice facilities, games facilities, concession facilities or any other facility (collectively referred to hereinafter as "Team Activities").

Releasing Parties hereby covenant not to sue Released Parties on account of any and all charges, claims, debts, disputes, demands, suits, causes of action, rights of action, dues, sums of money, accounts, liabilities, losses, expenses and damages, absolute or contingent, known or unknown, whether or not asserted, threatened, alleged or litigated, now existing or arising in the future, at law or equity, whether caused by the negligence of Released Parties or otherwise, that arise out of or relate in any way to Team Activities, and any claims for costs, expenses and attorneys' fees associated therewith.

In the event of a medical emergency, I hereby authorize to be transported to the nearest emergency room or medical facility. I also authorize officials to secure the use of an ambulance for transporting me to the hospital and/or to administer first aid treatment as necessary. I further authorize any physician, surgeon or dentist of the nearest emergency medical center to administer any emergency treatment procedure or medicine necessary or advisable. I further agree to pay the hospital, doctors and ambulance fees for all services rendered.
