MUST BE SUBMITTED BY THE TEAM CAPTAIN. Use this form only if the whole team is withdrawing from the Tournament. If less than all the team members are withdrawing and the team will add a new player(s), the incoming player(s) will reimburse the withdrawing player(s). Request must be submitted no later than March 13th, 2020 to be considered "No-Fault". All team members will receive their portion of the entry fee as soon as possible.
