Professor Consent Form

Complete this form to electronically grant consent to students seeking to enroll in a course. Complete the form in its entirety. Once the form has been submitted, students must visit the Enrollment Services Office within a week of the submission date to register for the course. Students seeking enrollment after a week of the submission date will not be permitted to register.

Please note: Ensure the correct name, identification number, and course number and section are entered into the form. Failure to enter accurate information may result in the student receiving a failing grade in the registered course.

No additional registration after deadline dates.

-New form must be submitted with each registration activity.
-Previous form cannot be reused if student is dropped from course for any reason.
