District Grant Final Report

Step 1 of 2: Submit

Congratulations! You have successfully completed your project. Now you need to get in all of the final reporting. Please complete this document and submit along with the information listed below. If you have any questions you can email rotary5130grants@gmail.com or call 707-337-2871.

District Grant funds will be disbursed to the club only upon acceptance of a complete Final Report by the District Grants Subcommittee.

Please refer to your original approved District Grant application!
Tip for Success: The person whose email is entered in the Primary Contact Email Address will receive notifications from FormSmarts with instructions on next steps during the review and final submission process.

Final Report

I hereby state that the information provided in this final report is complete and accurate.

Both the President and Primary Grant Contact have to electronically sign the grant report before submitting it for review. If one of you has not signed the report yet, click Save. The next screen allows you to forward the report to the other person for review and signature before you start the review process.

Steps for Successful Submission

Once you hit submit for review, you still have more steps to complete to finalize your report.

If your report has been rejected, you will receive a message from FormSmarts with a link to your report, your email, and a password. Go into the form, scroll to the bottom of the report, and review the additional information needed. Make the necessary changes to your report. Reinput the date for your electronic signature and re-submit for review.

If your report has been accepted, you will receive a message from FormSmarts letting you know that the report has been approved. This message will include a link to your report, your email, and a password. Go back into the form. Update the date field by your electronic signature and click on Continue. This will bring up a screen asking you to review your report. A box will appear at the top of this screen asking you to type in the characters as shown above the box. If the report looks complete, type the characters and press Confirm at the bottom of the page. Successful completion of this process will enable you to save your report as a pdf document.

Important:
The person whose email is entered in the Primary Contact Email Address will receive notifications from FormSmarts with instructions on next steps.

The Submit for Review process creates a temporary file that is open for 30 days. Action must be taken during that window to move the report to the next step. The final submission will create a permanent file with your grant report and all attachments.
