Lost or Damaged Shipment Notification

*Notice: IF YOUR SHIPMENT IS LOST OR DAMAGE AND YOU SIGNED THE DELIVERY RECEIPT AS A CLEAR DELIVERY WITH NO NOTATION OF LOSS OR DAMAGE, YOUR SHIPMENT IS NOW CLASSIFIED AS A CONCEALED DAMAGED SHIPMENT. YOU MUST IMMEDIATELY NOTIFY THE CARRIER SUBMITTING THIS NOTIFICATION DOES NOT NOTIFY THE CARRIER

Important Notice: All concealed/non-notated damages must be reported within 5 business days of delivery (1st day is the day of delivery) as per the NMFC (National Motor Freight Classification) - Rule 300100 - for a damaged claim to be filed. Reporting after this time will void a claim.
