THIS IS NOT A REGISTRATION The owner of the dam at the time of calving is responsible to report the birth of the calf with the IMZA. The dam should also be in the owner's name prior to calving. Please read the instructions regarding ID and Registration Rules on our website before completing this form.

USDA-AIN (This is a 15 digit form of ID that will always start with the numbers 840) NUES TAG (This is an ear tag that consists of 9 characters) Tattoo Microchip 15 characters or less (Please indicate the microchips location) Brand (Please give a short description and location of the brand) Ear Tag Color and Number - example: Green 1234 Primary ID (Select One Only. This is the ID that will appear on your printed certificate).

Sire and Dam Information : Please be sure the Dam is registered in your name before continuing. If the Sire and/or Dam are over 3 years of age it is required that their Permanent Adult Registration be completed before the calf can be reported.

You must include a current photo of the Miniature Zebu you are registering. Preferably a photo with the Dam if possible.
Mail the photo with this form. Please label the photo carefully with the animals full name on the back of the photo. Please do not staple the photo to the form.
