Please Submit this Request to Receive a Replacement Registration Certificate. Replacements will only be Issued to the Current Registered Owner Printed on the Original Certificate. No Alterations to any Recorded Information can be Made Under This Request.

IMZA Rule: 8
A duplicate certificate shall be issued upon a sworn statement to the IMZA from the recorded or registered owner of the Zebu that such certificate has been lost, destroyed or damaged. Duplicate certificates shall supersede the original. If the lost certificate is found after a duplicate was issued, it must be returned to the IMZA. The Submission of this form shall act as the Sworn Statement as required in IMZA Rule 8 of the Rules and Regulations adopted on July 10, 2021
