Submission of this form constitutes agreement to the Waiver and Authorization for use of Photo/Video Likeness. NOTE Enrollment in the Leagues is a commitment to show up and exercise good sportsmanship and agreement to adhere to the Safe Play Guidelines. Failure to do is grounds for dismissal and forfeiture of League fees.
