Sponsorship Form for Christmas Tree Lighting Event

To purchase a sponsorship, please complete this form:

• List your business’ name and address as you want it shown on the Christmas Tree Lighting posters, ceremony program, and news coverage.
• Indicate which sponsorship level you want.
• Each sponsorship level includes ornaments you may dedicate to someone if you wish; if you wish to dedicate the ornaments that come with your sponsorship level, indicate in the text box below to whom to dedicate them.

Each sponsorship level includes ornaments you may dedicate to someone if you wish (Pewter Sponsorship = 2 ornaments; Bronze Sponsorship = 5 ornaments; Silver Sponsorship = 15 ornaments; Gold Sponsorship = 30 ornaments; Platinum Sponsorship = 50 ornaments). If you wish to dedicate the ornaments that come with your sponsorship level, use the space below to let us know to whom to dedicate them. Please print the name of the person(s) you wish to honor (living) or remember (deceased) in the space provided below along with the address of the person who will receive the notification.
