Submission of this form constitutes agreement to the Waiver and Authorization for use of Photo/Video Likeness, adherence to Safe Play Guidelines, USTA ACES Pledge. NOTE Enrollment in the Leagues is a commitment to show up and exercise good sportsmanship and adhere to Safe Play Guidelines. Failure to do is grounds for dismissal and forfeiture of League fees.
