2026 MJD Hockey Skills Camp

Please complete all required fields to register for the 2026 MJD Hockey Skills Camp (July 6–9, 2026). Early bird pricing of $425 is available through April 30, 2026. Beginning May 1, 2026, the registration fee will increase to $450. After submitting your registration, please remit payment via cash, check (payable to MJD Hockey), or Venmo (@MJDHockey). Are you a returning camper? 
I, the undersigned parent/guardian, give permission for my child to participate in the 2026 MJD Hockey Skills Camp. I understand that ice hockey and related training activities involve inherent risks of injury., I voluntarily assume all such risks on behalf of my child and release and hold harmless MJD Hockey, LLC, its owners, coaches, staff, and the facility from any liability for injury or illness arising from participation.

I certify that my child is physically able to participate. In the event of injury or illness, I authorize camp staff to obtain emergency medical care if necessary and accept responsibility for related costs. I also grant permission for photographs or video taken during camp to be used for MJD Hockey promotional purposes.

By submitting this registration form, I acknowledge that I have read and agree to the above Waiver and Consent.
