Step 3 of the District Grant Process: Finally! You are qualified by the District to write a District Grant for 2024-25 Year. Follow the instructions below. For a simplified overview of the timeline check out the District Grant Checklist. APPLICATION DEADLINE: August 1st, 2024

Complete the application in its entirety. Both the Grant Primary Contact & Club President will be able to electronically sign and submit. All submissions of grant applications will be done electronically here - no paper applications accepted.

A few reminders….
  • Compliance with Rotary Foundation Requirements and Restrictions. As the qualification process makes clear, all district grant projects must adhere to the Terms and Conditions for Rotary Foundation District Grants and Global Grants. The most salient of these rules were summarized in the grants overview on which you were tested. Be sure to go back to that list if you have any doubt about whether your planned activity might violate one of the applicable rules. Remember that several of them relate to ensuring that Rotarians play an active role and enhancing Rotary’s image in the affected community.
  • District 5130 Requirements and Policies. While District 5130 is charged with ensuring compliance with the Rotary Foundation’s requirements and restrictions, it has also developed its own policies for the small grants it administers.

Proposed projects should fund new or expanded activities for the club and Rotarians must be involved in more than making decisions and writing checks. While district projects that can be completed within a limited period will sometimes be approved for a second or third year of funding, these grants are not intended to replace a set portion of the club’s budget on an annual basis. They are intended to provide seed money and allow clubs to experiment with new ideas.

In 2024-25 the maximum District Grant allowed per club is $2,200 as a 1:1 match for club contributions. Each club can use the entire amount for either a local project or an international project application. Or, it may be split between two smaller grants. *The grant is paid on a REIMBURSEMENT BASIS when final report is received and approved.

Will the project be completed by May 31, 2025? (due date for the Final Report is May 31, 2025) 
By signing this application, we certify that the project described in this application meets the requirements for a Rotary Foundation District Grant project as we understand them. We acknowledge that the District Grant funds will be disbursed on a reimbursement basis only after the Final Report is accepted by the District Foundation Grant Subcommittee as complete. If for any reason the project is changed or abandoned, we will promptly notify the Grants Subcommittee Chair at rotary5130grants@gmail.com
INSTRUCTIONS FOR APPLICATION SUBMISSION: (PLEASE READ CAREFULLY)

1. Make sure all required fields above are complete.
2. Complete the fields below for BOTH the Club President Name/ Email and the Primary Contact Name/Email
3. Electronically Sign and Date whichever role you are
4. Click the blue SAVE button. You can come back and edit/save the document with changes for up to 30 days if needed. NOTE - It will generate a new password*)
5. Once you are ready for submission - On the next screen SHARE this document with the other signing party. They will receive an email to open, electronically sign and SUBMIT your application.
6. Your report will be accepted or rejected during the review process. If your report has been rejected, you will receive a message from FormSmarts with a link to your report, your email, and a password. Go into the form and review the additional information needed. Make the necessary changes, reinput the date for your electronic signature and re-submit for review.
If your report has been accepted, you will receive a message from FormSmarts with a link to your report, your email, and a password. Go back into the form and update the date field by your electronic signature and click on Continue. This will bring up a screen asking you to review your report. Note: a CAPTCHA box may appear at the top of the screen asking you to type in the characters as shown above the box. If the report looks complete, press Confirm. You will receive an acknowledgement from FormSmarts.
7. Important:
The person who submits the application or final report will receive notifications from FormSmarts for next steps.
The Submit for Review process creates a temporary file that is open for 30 days. Action must be taken during that window to move the report to the next step. The final submission will create a permanent file with your grant report and all attachments.
