FormSmarts Terms of Use

1. The registration and use of FormSmarts web form service are offered free of charge.
2. None of the services provided by FromSmarts may be re-sold or provided to a third party, unless explicitly authorized by FormSmarts.

Acceptable Use

1. FormSmarts may not be used for websites containing adult content, promoting racism, involved in gambling, spamming, scams, and more generally illegal activities.
2. Each user, whether a company or individual, may only create up to five FormSmarts accounts. Permission to create more accounts may be granted by contacting FormSmarts.
3. We reserve the right to suspend web form accounts for which resource usage is deemed by us to high (if you believe this may apply to you, contact FormSmarts Support).
4. Information submitted via web forms is transmitted over standard, non-secure web protocols, and we therefore prohibit the use of FormSmarts to communicate confidential information such as credit card details.

Use of Form URLs

Each web form can be accessed by a unique URL on FormSmarts website. The following restrictions apply to the publication of such URLs.

1. Form URLs may not be published within opt-in bulk emails without prior authorization. The authorization will only be granted to verified form owners.
2. Form URLs may not be published within unsolicited and opt-out bulk emails.
3. Form URLs may not be published on internet forums or newsgroups where this could be seen as off-topic or spam.
4. The same restrictions apply to any URLs redirecting to a web form.

Account Termination

1. Users may terminate their FormSmarts account by deleting all the web forms on the account. Inactive accounts with no forms associated will be removed.

Disclaimer

1. We provide this service “as is”, in the hope it will be useful to you, but without any warranty; without even the implied warranty of merchantability or fitness for a particular purpose.
2. We reserve the right to withdraw part or all of the service at anytime, without notice.
