Show a Form Owner Name on Your Forms
Increase user confidence by letting us verify you own the websites you are using with FormSmarts. FormSmarts will then display a verified form owner name in the top-right corner of all your forms.
To verify your sites, sign in to FormSmarts, and choose the Verified Websites option in the Settings tab. The form owner name can currently only be a verified web hostname. We will soon also support verified personal or company names. More details here.

How can i remove the owners name/website of my form? :)
Andrew,
Once you’ve verified your website we will show it on all your forms to let form users know they’re sending information to you. If don’t want the form owner to appear on your forms, you can edit change the return URL to point to a website you haven’t verified. For example, if the return URL is “example.com/thankyou.html”, you can change it to “www.example.com/thankyou.html”.
If you’re a Pro user, you can also contact FormSmarts Support.
how can i delete the verified url? not just replacing it with a non-verified website? :) can i still reuse the website that i replaced? :) thanks
Andrew,
There is no way to “un-verify” a website for Standard users at this time. Yes, you will still be able publish the form on that site, even if you’ve changed the Return URL to another website (e.g. http://formsmarts.com).
